The Peabody Resident Services (PRS) Home Care (GAFC) Program established in 1995 is designed to serve residents of affordable housing with the goal of providing the support services they may need to help them to remain living in their own homes. PRS is one of the largest Medicaid Group Adult Foster Care providers in Massachusetts, serving approximately 1750 elders and persons with disabilities in over 500 buildings throughout the state in PPI managed and non-PPI managed buildings.
What we do
- Assist elders and persons with disabilities to remain at home and in the community
- Provide assistance with activities of daily living (ADLS) and instrumental activities of daily living (IADLS) to eligible individuals
- Offer services that include: personal care, housekeeping, laundry, grocery shopping, meal preparation, medication cueing, transportation coordination, comprehensive case management, and wellness nursing visits
Program Eligibility
Program participants must meet ALL of the following criteria:
- 22 years of age or older
- Insured by MassHealth Standard or other specified Senior Care Options and One Care Plans
- In need of daily assistance with personal care
- Approved by their physician and healthcare plan for personal care services
- Reside in “Clustered Housing” (i.e. subsidized apartment building)
